What is considered Chronic Care? 

1) the insured is unable to perform, without Substantial Assistance from another individual, at least two or more of these Activities of Daily Living for an expected period of at least 90 days, due to loss of functional capacity:

  • Bathing 
  • Eating
  • Continence 
  • Toileting
  • Dressing  
  • Transferring

Or

2) the insured has a Severe Cognitive Impairment that requires Substantial Supervision to protect the insured from threats to health and safety.

A Licensed Healthcare Practitioner is defined as a Physician, a registered professional nurse or licensed social worker who is not a member of the insured’s or policy owner’s immediate family.
